Welcome Week Waiver, Indemnity and Consent Form

In consideration of being permitted to participate in Saskatchewan Polytechnic Welcome Week activities (the “Activities”) and use the associated recreational/entertainment equipment (the “Equipment”) provided by Saskatchewan Polytechnic (“Sask Polytech”), located at its campus locations, the undersigned individual hereby confirms they are an adult and they acknowledge and agree to the following terms and conditions:

1. Assumption of Risk: The undersigned acknowledges that participation in the Activities and the use of the Equipment involves inherent risks, including but not limited to the risk of injury from bouncing, falling, colliding with other users, exposure to food allergies, insects, other outside environmental elements,  and other unforeseen circumstances. The undersigned voluntarily assumes all such risks.
2. No Supervision: The undersigned agrees that their participation in the Activities and use of the Equipment may be unsupervised and that at all times the undersigned is expected to behave and govern themselves as a responsible adult.
3. Health and Fitness: The undersigned warrants that they are in good health and physically fit to participate in the Activities and use the Equipment. If any medical conditions exist that might increase the risk of injury, the undersigned agrees they will not proceed without having consulted a physician.
4. Rules and Guidelines: The undersigned agrees to adhere to all rules and guidelines provided by Sask Polytech or an Equipment provider for the safe use of Equipment. This includes restrictions on the number of participants, proper footwear and attire, removal of sharp objects, and any other instructions given.
5. No Food or Drink: The undersigned agrees not to bring any food, drinks or chewing gum onto the Activities sites or Equipment in order to prevent choking hazards, reduce risks and maintain a clean environment.
6. Prohibited Activities: The undersigned agrees not to engage in any prohibited activities on the Equipment, including but not limited to climbing the Equipment, wrestling, somersaults, flips, or any other actions that could increase the risk of injury.
7. Liability Release: The undersigned releases and holds harmless [Company Name], its employees, agents, and representatives from any and all claims, liabilities, injuries, losses, or damages arising out of or in connection with participation in the Activities or the use of the Equipment, including any injuries resulting from negligence.
8. Indemnification: The undersigned agrees to indemnify and defend Saskatchewan Polytechnic against any claims, liabilities, or expenses, including legal fees, arising from participation in the Activities or the use of the Equipment by the undersigned.
9. Emergency Medical Treatment: In the event of an injury or medical emergency, the undersigned grants permission to Sask Polytech to seek emergency medical treatment for the undersigned, as deemed necessary.
10. Photography and Publicity: The undersigned acknowledges that photographs or videos may be taken during the use of the bouncy castle and grants Sask Polytech the right to use such media for promotional purposes.
11. Other Agreements: This waiver, release and consent is in addition to and does not replace any other agreements between the undersigned and Sask Polytech.

I, the undersigned, have read and understood the above waiver and agree to abide by its terms. I am aware that by signing this waiver, I am releasing certain legal rights that I or my legal representatives may have against Sask Polytech.
