In order to receive services, students must present documentation of their disability to Accessibility Services. Accessibility Services has a verification of disability form (pdf) that can be provided to the health care professional to complete. Depending on the accommodation needs, our office may ask for additional, updated, or more specific documentation.
Documentation required
Documentation from a registered health care professional is required, as outlined below.
If you have/are:
an acquired brain injury
ADHD
Autism Spectrum Disorder
blind or have low vision
deaf, deafened or hard of hearing
a learning disability
You need to provide a current psycho-educational assessment completed by a registered psychologist.
a medical disability
a mental health diagnosis
a physical disability/mobility impairment
Accessibility Services may also be able to provide support to a student who is experiencing a disability that is temporary in nature. Documentation that identifies the duration of the disability is requested.
